10.04 Eye Care Program:
It is agreed that the University shall continue the Eye Care Program during the period of this Agreement. The program shall be administered by the University and shall provide benefits to all eligible staff and their eligible dependents (spouse and unmarried children under twenty-three (23) years of age who live with the staff member in a regular parent/child relationship).
The program shall provide for eligible staff and dependents to receive a thirty-five ($35.00) dollar payment for prescription eyeglasses with regular lenses or a forty ($40.00) dollar payment for such glasses with bifocal lenses. The program shall provide thirty-five ($35.00) dollars per exam annually and/or exam to be provided at campus.
Eligible staff and eligible dependents (as defined above) shall be eligible for a maximum payment of a thirty-five ($35.00) dollars or the cost, whichever is less, for an eye examination by an Ophthalmologist or an Optometrist.
Each eligible staff member and dependent shall be eligible to receive only one(1) payment for glasses and one (1) payment for examinations during the fiscal periods of July 1, 1999 to June 30, 2001. The extension of benefits to dependents shall be effective only after the staff member has been continuously employed for a minimum of sixty (60) days. Proper affidavit and submission of receipts are required of the staff member in order to receive payment and must be submitted to the local Human Resources or Benefits office. The program will discontinue after June 30, 2001.
10.05 Life Insurance Program:
Life insurance coverage is provided as part of the Public Employee Retirement System (PERS), or the Alternate Benefit Program. Both programs are administered by the New Jersey Division of Pensions. Eligibility for participation by staff members and benefits are governed by statute and Rules and Regulations promulgated thereunder and administered exclusively by the New Jersey Division of Pensions.
10.06 Pension:
The University is a participant in the Public Employee Retirement System (PERS) and the Alternate Benefits Program. Eligibility for participation by staff members and benefits are governed by statute and Rules and Regulations promulgated thereunder and administered exclusively by the New Jersey Division of Pensions. A written description of the PER Program or Alternate Benefits Program can be obtained from the University’s Benefits or Campus Human Resources Office.
